Immediate vs Delayed Loading: How Soon Can You Have New Teeth?

What loading actually means

"Loading" refers to when a crown or bridge is attached to an implant and put into function. Delayed loading waits until the implant has fully fused with the jawbone, a process called osseointegration, before any permanent tooth is fitted. Immediate loading attaches a temporary fixed tooth on the same day as surgery, well before that fusion is complete.

The traditional, delayed approach

In a delayed protocol, the implant is placed and left to heal undisturbed, typically for three to six months, before impressions are taken for the final crown or bridge. This remains the most predictable route for many single-tooth and multi-tooth cases, particularly where bone quality is average or the site needed grafting.

Immediate loading and same-day teeth

For suitable full-arch cases, such as All-on-4, immediate-loading techniques allow a fixed temporary bridge to be attached to the implants at the same visit as surgery. In the right cases you can leave with fixed teeth in place within the same trip, then return later for the definitive bridge once healing is complete.

This is not a shortcut available to everyone. It depends heavily on the implant achieving strong initial stability in the bone at the time of placement, confirmed through careful 3D planning rather than assumed.

What determines which approach suits you

  • Bone density and volume at the implant site, confirmed by a CBCT scan
  • How much initial mechanical stability the implant achieves during placement
  • Whether grafting was needed at the same site
  • Your bite forces and any grinding habits
  • How many implants and teeth are involved

Why an honest clinic will not always offer immediate loading

Immediate loading, done in the wrong case, risks micro-movement of the implant before it has properly fused, which can jeopardise the whole result. At Dentalmed, our clinical team only recommends immediate loading when your scan genuinely supports it, and will say plainly when a staged, delayed approach gives you a better long-term outcome.

Questions

Frequently asked questions

Can any implant be loaded immediately?

No. Immediate loading depends on the implant achieving strong initial stability at placement, which in turn depends on bone density and volume. Suitability is always confirmed from a 3D scan, not assumed.

Is immediate loading riskier than delayed loading?

When used in a well-selected case, immediate loading has a good track record, particularly for full-arch restorations. Used in the wrong case, it does carry more risk, which is why careful case selection matters more than the technique itself.

Do I get temporary teeth either way?

For single implants under a delayed protocol, a temporary solution is often provided while the implant heals. For immediate-loading full-arch cases, the temporary bridge is fixed rather than removable from day one.
