Why bone is lost in the first place
Once a tooth is lost, the bone that used to support it starts to shrink, sometimes losing a meaningful proportion of its width within the first year alone. Long-term denture wear, gum disease and old extractions can all leave a ridge too thin or too short for an implant to hold securely.
In the upper back jaw there is an additional complication: the maxillary sinus, an air-filled space that often sits low over the molar roots, further limiting the bone available for an implant.
Socket preservation
When a tooth is removed, graft material can be placed directly into the empty socket to slow the natural shrinkage that follows and keep the ridge ready for a future implant. It is the simplest and most commonly used form of grafting.
Sinus lifts
A sinus lift gently raises the sinus membrane in the upper back jaw and packs graft material beneath it to build the vertical height an implant needs. A smaller "internal" lift can sometimes be carried out at the same time as implant placement, while a larger "lateral window" lift usually needs to heal before an implant is placed.
Guided bone regeneration
For a thin or narrow ridge, graft granules can be shaped over the area and covered with a protective collagen membrane that holds the material in place while your own bone grows through it. This technique, along with regenerative approaches such as platelet-rich fibrin (PRF) prepared from your own blood, supports predictable healing.
How long healing takes
- Socket preservation and minor bone regeneration: roughly three to four months
- A larger sinus lift: commonly around four to nine months before implants are placed
- Minor grafting: sometimes combined with implant placement in the same appointment
- Larger augmentations: staged, with the graft healing fully before the implant is placed
