Signs You Might Need a Dental Crown

Where a filling stops being enough

Fillings work well for small to moderate cavities, but once a tooth has lost too much of its own structure, through deep decay, a crack, or a filling that has failed one time too many, it needs a different solution. A crown covers the whole visible tooth, restoring strength as well as shape.

The decision is not just about size. It is about whether enough healthy tooth remains to hold a filling securely under normal biting forces.

Common signs to look out for

  • A visible crack or fracture line, especially one that reaches the gum
  • Sharp pain when biting down, which can indicate a hidden crack
  • A tooth that has just had root canal treatment, since these teeth become more brittle over time
  • A large old filling that is breaking down or has decay underneath it
  • A tooth that has darkened or changed shape after an injury

Crowns after root canal treatment

A root-treated tooth is no longer fed by its own blood supply, which can leave it more prone to fracture, particularly at the back of the mouth where chewing forces are highest. For this reason, a crown is often recommended shortly after root canal treatment rather than left until a problem develops.

This does not apply to every root-treated tooth. Front teeth with less biting load sometimes do fine with a more conservative restoration, and your dentist should explain the reasoning either way.

Cosmetic reasons for a crown

Crowns are not only used to rescue a damaged tooth. They are also placed on healthy teeth to correct significant discolouration that whitening cannot reach, or to reshape a tooth that is unusually short, worn or misaligned within an otherwise even smile.

How the diagnosis is made

A proper diagnosis starts with an examination and, where needed, a CBCT scan or X-ray to see what is happening beneath the surface. At Dentalmed, our clinical team only recommends a crown when the diagnostics genuinely support it, never as a default response to a damaged tooth.

If you are unsure whether your tooth needs a filling, a crown, or something else entirely, send us a recent photo or X-ray and we will give you an honest recommendation before you travel.

Questions

Frequently asked questions

How do I know if I need a filling or a crown?

The general rule is that a filling suits smaller cavities where enough healthy tooth remains, while a crown is needed once decay, a crack or a previous filling has left too little structure to support a simple repair. A dentist can confirm this with an examination and, where needed, an X-ray.

Do all root canal treated teeth need a crown?

Most back teeth benefit from a crown after root canal treatment because they can become more brittle over time, but not every case is identical. Front teeth with lighter biting forces sometimes need a less extensive restoration instead.

Can a crown be placed on a healthy tooth?

Yes, crowns are sometimes used for cosmetic reasons, such as correcting a tooth that is significantly discoloured, unusually short, or misshapen compared with its neighbours.
